Program Limitations and Exclusions

  1. This plan cannot be combined with other specials, offers, or discounts.
  2. For DEEP CLEANINGS, procedures are in the periodontal category and are covered under the 10% discount.
  3. This plan cannot be used  with or in conjuction with a dental insurance policy.
  4. Annual membership fees are non-refundable.
  5. 10% discount on procedures performed by general dentist. Specialist may not be included with this discount.
  6. This does not cover loss or theft of dentures.
  7. Benefits can not be transferred to other family members.
  8. This is a dental discount plan and is NOT a dental insurance plan.
  9. No Refunds will be given if the patient chooses not to use the dental plan.
  10. All payments are due at the time of service in order to receive the discount.
